Non anonymous survey answer

Add a configuration on the survey module that allows you to perform non-anonymous surveys, so that user names and email can be present in the export. So that it's completely transparent to the user, when the config option is selected, the survey page will show user data fields already filled so that it’s clear that the account data info is shared along the survey response. MEL is ready to finance this.
